Medical Requirements
Physical Examination and Immunizations: A physical examination and immunizations are required for students new to Illinois, Early Childhood, Kindergarten (or entering school for the first time), sixth grade, and ninth grade. For all students entering twelfth grade and at least 16 years old, a second dose of meningococcal vaccine is required (unless the first dose was given after the age of 16). The physical examination and immunizations must be dated within one year from the first day of school and must be completed on the State of Illinois Certificate of Child Health Examination form (not the Sports Physical form), by a licensed Healthcare Professional.
Unless an exemption or extension applies, proof of the required physical examination and immunizations must be submitted by October 1st. If not submitted by October 1st, the student will be excluded from school until the required proof of a physical examination and immunizations is submitted. Transfer students will have until October 1st or 30 days from the first day of student attendance, whichever is the latter, to submit proof of the forms or the student will be excluded from school.
Dental Examination: The dental examination form is required for students in Kindergarten, second grade, sixth grade, and ninth grade. Dental examinations must be dated within 18 months of the May 15th due date for the current school year and be completed on an Illinois Department of Public Health Proof of School Dental Examination form. The dental examination form is due by May 15th.
Eye Examination: The vision examination is required for students in K (or entering school for the first time), or are new to the state of Illinois. Vision examinations must be dated within one year from the first date of school and must be completed on a State of Illinois Eye Examination Report form by a licensed ophthalmologist or optometrist. The Eye Examination Report form is due by October 15th.
Vision and Hearing Screenings: Vision screenings are mandated by the Illinois Department of Public Health for students in Preschool, Kindergarten, second grade, eighth grade; students that receive special education; students who are new to the district; or upon teacher referrals. At the start of the school year, notify the school nurse if your child has a completed eye examination within one year from the first day of school by an ophthalmologist or optometrist or if you do NOT want your child screened. Notification is required annually.
Hearing Screening: Hearing screenings are mandated by the Illinois Department of Public Health for students in Preschool, Kindergarten, first grade, second grade, and third grade; students that receive special education; students who are new to the district; or upon teacher referrals. At the start of the school year, notify the school nurse if your child has a completed ear examination including an audiological evaluation completed within one year from the first day of school by an audiologist or if you do NOT want your child screened. Notification is required annually.
Medication: If it is necessary for a student to receive medication during school hours (including over-the-counter medication such as cough drops), a School Medication Administration Authorization form, which includes parent/guardian permission and orders by a licensed Healthcare Professional, MUST be on file with the health office for the student to receive medications. The form is available on the district website at Health Services or from the health office. Both sections of the form must be completed before school personnel can dispense any medication. A new form is required annually.
Medications are to be dropped off to the health office by the Parent/Guardian in the original labeled container from the pharmacy. Over-the counter medication must be in the manufacturer’s original label intact with the student’s name added to the container. Students are expected to go to the health office to take their medication.
Emergency Medication: A School Medication Authorization form with the authorization for the student to Self-administer and/or Self-Carry Emergency Medication completed by the parent/guardian and the licensed Healthcare Professional must be on file in the health office by the first day of attendance. This authorization form allows the student to self-administer and/or self-carry the following emergency medications: Asthma medication, Epinephrine injector, Diabetic management medication, or Seizure emergency medication.
