5Essentials Parent Survey
As a parent, you now have an opportunity to share your thoughts on the important elements of school effectiveness in a survey about your child’s school called the Illinois 5Essentials Survey.
Why the 5Essentials Survey
On behalf of the Illinois State Board of Education, the Illinois 5Essentials Survey will be administered online by UChicago Impact at the University of Chicago. The 10 minute survey gathers data related to five indicators that can predict important student outcomes, including improved attendance and larger test score gains. These five indicators that affect and predict school success are:
- Effective Leaders
- Collaborative Teachers
- Involved Families
- Supportive Environments
- Ambitious Instruction
Prior research in more than 400 schools has shown that schools that were strong on at least three of these “5Essential” indicators were 10 times more likely to improve student learning gains in math and reading than those that were weak on three or more of the 5Essentials indicators.
5Essentials Parent Survey (20% Participation Needed)
Yorkville CUSD 115 has chosen to provide the optional Parent Supplement survey. If at least 20 percent of Yorkville CUSD 115 parents complete this survey, a parent supplement report will also be generated. The survey takes approximately 10 minutes. The parent data will not be reported on the State Report Card.
Your participation in the parent portion of the survey will help us understand the conditions at your child’s school and guide improvement. If a parent has more than one (1) child and they attend the same school, that parent may elect to participate multiple times. For parents with children enrolled in multiple schools, we recommend that they participate in the survey for each school. Only one parent per household may submit a survey for a given child. NOTE: Your identity and survey responses will be kept completely confidential and will never be connected to you or your child.
The Illinois 5Essentials Survey window for the Parent Survey Supplement is now open and will close on March 11, 2022. We need 20% of our parents to participate so please complete the survey today!

1) Once you arrive at the Welcome Portal please select the Parent Survey.
2) Fill in the information requested and click Login.
5Essentials Student Survey
Students across Illinois will also have an opportunity to participate in the statewide Illinois 5Essentials Survey. This survey is designed to generate a detailed picture of the inner workings of your child’s school.
Every year, districts across Illinois participate in the Illinois 5Essentials Survey. All 4th through 12th grade students will be responding to this survey. 5Essentials Reports will be generated for schools if their students meet the response rate threshold of 50 percent. These reports will be sent to schools and districts in April 2022, and will also be included in the State School Report Card.
Students’ identities will be completely confidential. Teachers or administrators will never see individual responses to survey questions and will only receive aggregated information, such as the total percentage of students who agree homework assignments help them learn the course material.
The Illinois 5Essentials Survey window for the Student Survey is now open and will close on March 11, 2022. The student survey takes about 25 minutes to complete and students will take the survey on a specific date determined by their individual school and promoted through their school newsletter.
