Y115 Facilities Usage Instruction for Requester

Please see instructions below on how to begin the process of requesting the usage of district facilities.

  1. Email a valid Certificate of Insurance to Sam Anderson at sranderson@y115.org.  

  2. Navigate to the Community Use site.  

  3. In the upper right hand corner, click “Log in to Request Facility Use” and create an account.

You will receive an email once you are approved to begin requesting the use of a facility. If you need assistance, reference the “Documents” tab on the Community Use site.

After you submit a request to use a facility, you will receive an automated email that your request has been submitted. Once the request has been reviewed, you will receive an email notification indicating if the request was approved or denied.
